Investigation Summary
On April 15, 2011 an employee was standing on the roof of a building as a metal waste bin was being set on the roof by a forklift truck. The forks of the lift truck were tilted and the tips of the forks caught between two boards. The metal bin bounced off the forks of the forklift truck when the operator attempted to free the forks. Employee was struck in the face by the metal bin. Employee's jaw was fractured. The employee was a drywall contractor.
