Investigation Summary
On June 1, 2004, Employee #1, a grader at a sawmill that specializes in fencing products, was working at the remanufacturing chain, pushing a unit of fence boards out to be picked up by a forklift. Across from Employee #1 was a banding stanchion where a forklift was located. Employee #1's back and the rear of the forklift were facing each other. For some reason, the forklift operator began backing the vehicle away from the banding stanchion toward the remanufacturing chain that Employee #1 was working at. Both the forklift operator and Employee #1 were unaware of each others locations. The counter-weight of the forklift struck Employee #1's left foot, fracturing his ankle, and causing him to fall to the ground. Employee #1 was hospitalized and treated for his injury.
