Investigation Summary
A crew of employees was working to frame and plate walls at this construction site. Employee #1 (the supervisor) asked the other employees at the site to help him raise the wall he had been framing. This wall was a balloon wall constructed of 2-in. by 6-in. boards, and had four windows with the headers installed. A total of nine people attempted to raise this 16-ft wide and 18-ft 2-in. high wall. Two employees were holding 2-in. by 4-in. by 16-ft braces that were attached at a point 11 ft up the wall and were standing outside of the wall on the soil. The other seven employees were at the top plate and began to push up on the wall. They got the wall at an approximate 45-degree angle and could not go any higher. This wall was too heavy to be manually raised by nine people. The employees were lifting and holding the wall and standing in the middle of the wall while Employee #1 was attempting to place braces under the wall but the employees who were holding the wall began to lose control and the wall fell. Employees under the wall were able to get into the window openings except for Employee #1 who was struck while in the middle of the wall. Employee #1 was killed when the entire wall came down on top of him.
