Investigation Summary
On June 23, 2008, an employee, a laborer, was operating a moulder surfacing the sides and face of the lumber. The moulder jammed. The employee opened the cover over the heads of the moulder to clear the machine. He did not lockout the moulder before reaching in to clear the moulder head. The employee's right hand contacted the turning moulder head as he pulled a piece of lumber out of the moulder head. The employee's little finger on his right hand was amputated when it contacted the turning moulder head. The employee was hospitalized and treated for finger amputation.
