Investigation Summary
On January 25, 2008, Employee #1 of Roseburg Forest Products was preparing to load peeler core on to the in-feed chain to the chipper. A surplus of peeler core was being chipped to produce wood chips. Employee #1 stood at the base of a unit of peeler core that was stacked three units high. Employee #1 cut the banding on the bottom unit of the stack of peeler core. This allowed the bottom unit to spread and caused the entire stack to tilt and fall. Employee #1 was pinned by the falling units of peeler core.
