Investigation Summary
On January 25, 2008 Employee #1 a laborer worker of Unity Forest Products was at the out feed chain conveyor behind the trim saws at number 1 planer. Employee #1 was pushing short boards to the other side of the conveyor. Employee #1 noticed an end trim at the end of the trim saw lug chain conveyor. Employee #1 reached over to flick the piece of wood away from the end of the lug chain. A lug on the chain conveyor pinched Employee #1's little finger between the small end trim and the lug of the conveyor chain. The tip of Employee #1's right little finger was amputated at the hospital, due to the crushing injury.
