Investigation Summary

Investigation Nr: 170871644
Event: 01/07/2002
Employee injured in fall from ladder

On January 7, 2002, Employee #1, a Vann Brothers Shop Manager, was going to get some hoses and fittings out of a storage shed. He instructed a coworker to get a forklift and meet him at the shed. When Employee #1 entered the shed, he noticed that hoses and fittings were located on the storage deck, approximately 8 feet high. He used a 9-foot metal ladder to access the parts located in bins on the storage deck. Before he could get on the deck, the ladder slipped and Employee #1 fell between 7 and 8 feet to the concrete floor. He was later found by his coworker lying on the floor semi-conscious. Employee #1 suffered serious head and neck injuries and was transported to UC Davis Medical Hospital in Sacramento.
