Employee's finger amputated in rolls of forming machine

At approximately 1:20 p.m. on January 29, 1998, Employee #1 was cleaning a sheet metal forming machine. He was sanding mold from the forming rolls with the machine running when his left hand became caught and was pulled into the rolls and crushed. Employee #1's middle, ring, and small fingers were amputated, along with his index finger from the mid-knuckle and the tip of his thumb. He was hospitalized for four days.
