Investigation Summary
At approximately 9:00 a.m. on September 5, 2001, Employee #1 was cutting wooden stakes for a concrete form out of a piece of 2 ft by 8 ft lumber. Working off of a saw horse he was using a skil saw. Holding the wood in his right hand and the skill saw in his left, he cut into a knot. A piece broke off causing the saw to jam and kick back. When the saw kicked back it came across the back of his right hand and wrist, causing a severe laceration and he was hospitalized.
