Employee Injured When Arm Caught In Machine

On February 20, 2001, Employee #1 was directed by his supervisor to operate the SWF (case sealer) Quickset Machine. The SWF Case Sealer Machine is part of a sequence of several machines that package products. Small bags of fruit are placed in cardboard boxes and at the SWF Case Sealer Machine the box is sealed with glue. Up to four cases per minute are processed. The accident occurred when Employee #1 placed his left arm in an unguarded hazardous area of the SWF Case Sealer Machine to retrieve a small bag (package). The bag was underneath the machine on the outflow side. Employee #1 was on the south and outflow side when he bent down on his knee and reached into the hazardous area of the machine while the machine was still running. Employee #1's arm was caught in the machine in a pinch point created by the push bars on the revolving chain conveyor and the structural member of the machine. Employee #1's injuries included multiple fractures of his arm and a separated shoulder. Employee #1 was hospitalized.
