Investigation Summary
Employee #1, a fuel delivery tank truck driver with 17 months of experience, climbed onto a catwalk at the top of a 10,000 gallon diesel fuel tank and saw that it was about to overflow. He was scrambling down the tank access ladder to shut off the fuel pump on the truck when he lost his footing and fell 6 to 8 ft onto the concrete floor at the base of the ladder. Employee #1 fractured one or more bones in the heel of his left foot.
