Employee injured in fall from scaffold outrigger shelf

Employee #1 was erecting outriggers on an existing scaffold so that it could accommodate blocks for bricklayers. He first installed the frame of the outriggers. He then stood on the outrigger shelf, which was comprised of two 2 in. by 10 in. planks, and began moving the scaffold planking into place. Employee #1 lost his balance and fell approximately 13 ft to the ground. He sustained head and neck injuries, for which he was hospitalized. The accident occurred early in the morning and there may have been ice on the planks.
