Investigation Summary
Employee #1 was assigned to clean a raisin vacuum cleaner on line #10. He was cleaning the upper chamber when he either stepped in or slipped into it. Employee #1's left foot became caught in the rotating auger at the bottom and was pulled in. He sustained severe injuries and his foot had to be surgically amputated. Employee #1 had not de-energized the vacuum system before starting to clean it.
