Investigation Summary
Employee #1 was feeding wire stock into a wire crimper. He was having problems with the wire feeding properly, so he raised the guard and tried feeding the wire next to the crimping wheels. Employee #1 was trying simultaneously to feed two wires, keep them separated, and jog the machine, when the gloves he was wearing became snagged by the wire. His left hand was pulled into the crimping wheels, amputating the tip of his finger.
