Employee's fingertip amputated when caught in hopper valve

Employee #1 was cleaning out the paint hopper on the bottling line in preparation for a color change. He began by rinsing out the hopper from the top as he'd been instructed. He was rinsing the valve at the bottom of the hopper when the spray nozzle somehow became caught in that area. The valve cycled while Employee #1's right hand was near it, catching one of his fingers and amputating the tip.
