Investigation Summary
Employee #1 was making kitchen cabinet doors. He was running an outside detail on a cabinet door using a Pacco shaper. He had approximately 6 inches left on the run. There was no guard on the machine and he was not using a push stick of any type. Employee #1's hand slipped and contacted the shaper blade, amputating the small finger of his right hand and fracturing his ring finger in three places. Employee #1 was hospitalized.
