Employee's fingertip amputated in palletizer

Employee #1 was operating a palletizer when the stacker unit began to malfunction. His foreman told him to turn off the machine and then to reach inside and grab the microswitch to check if it was loose. Employee #1 followed these instructions but when he grabbed the switch, the machine cycled. The tip of his right index finger was amputated.
