Investigation Summary
At approximately 1:30 on December 28, 2004, Employee #1 was adjusting a chain on Birght Coop Chicken Loader Number 204. The chain was located under a panel at the end of the conveyer where the chickens were placed into storage units for transport. Employee #1 stated that the loader operation key was in the off position and the device was idling while he was performing the adjustments. The chain could be activated by a button at the control panel or by a trip sensor, which allowed the chain to move one cycle. Employee #1's hand was under the chain making minor adjustments when the chain activated, amputating four of his fingers. Employee #1 was hospitalized and treated for his injuries.
