Investigation Summary
Employee #1 was installing sheathing on the roof of a warehouse. Employee #1 got up to get a drink of water. When Employee #1 returned, he fell through a hole where the sheathing had been removed earlier. Employee #1 fell 24 ft to the ground and he suffered a fractured hip, fractured wrists and internal injuries. Employee #1 was hospitalized.
