Investigation Summary

On February 10, 2005, an employee was standing 1 or 2 feet off floor on 6-ft wood step ladder removing shop vacuum from 5-ft shelf, when he fell backwards landing on his buttocks. He was killed. It was determined that the cause of death was due to a massive acute right subdural hemorrhage over a period of days, a rebleeding of old chronic subdural neomembrane, and a remote blunt head trauma suffered during a fall on tennis court on July 14, 2002. It was not due to the incident involving fall from ladder on February 10, 2005.
