Employee Struck in the Head When Cable Snaps

On July 19, 2005, Employee #1 was stripping formwork from concrete. He had a come-along attached to the form and to a steel rod embedded inside the concrete wall. The Formwork was located overhead and a tower crane supported the weight of the form. He was cranking the come-along and the steel rod, located in the concrete wall, snapped and struck him in the head. He sustained a fractured skull and was hospitalized.
