Investigation Summary
At approximately 7:00 a.m. on June 28, 2004, an employee was removing debris left over from the previous day on the roof so he could begin his morning assignment. The employee had been instructed to construct a parapet wall on the roof. The employee removed an inch plywood board over an access hole in the roof just before the accident. Evidence suggests that the inch plywood board was previously nailed down to 2-in by 4-in. lumber surrounding the hole. For some unknown reasons the nails had been removed, except in one corner, before the employee attempted to remove the inch plywood. At the time of the accident the employee, while sliding the inch plywood to the side with a twist of his body, failed to realized the plywood covered a hole in the roof. Upon removing the plywood the employee walked backwards into the hole resulting in a 14-ft 6-in. fall to the concrete below. The employee was taken to V.A. hospital in Lancaster, where he was hospitalized. He suffered a broken wrist, requiring three surgeries and a fractured back.
