Investigation Summary
At approximately 10:20 p.m. on August 17, 2004, Employee #1 was working as a maintenance repair person on a polystyrene injection press (press) at Styrotech Inc. He was repairing the lid-lock component of the injection press. This maintenance activity was completed on the front side of the press. After completing the repair work, Employee #1 removed his lock and energized the press to test the machine. The machine immediately became jammed with a polystyrene container between the hydraulic driven metal molds. In response Employee #1 climbed up the stairs on the side of the press and reached his hand in where the polystyrene container was stuck. The polystyrene container became unstuck and the machine activated. Concurrently, Employee #1 became distracted by a coworker calling out to him thereby turning his head away from his hand. Employee #1's right hand was crushed between the metals molds with approximately 2500 PSI of force. Employee # 1 suffered a complete amputation of his right hand. He was initially treated at the Delano Hospital and immediately moved to the University of California Medical Center in San Francisco for treatment.
