Investigation Summary
Employee #1 was adjusting belt on feeder sprocket, when the first joint of his index finger and the tip of his middle finger was amputated. Employee #1 failed to use lockout/tagout procedures prior to adjusting belt on feeder sprocket. The Employer operated an egg ranch, and at the time of the accident, Employee #1 worked for the employer for approximately 25 years. Employee #1 was hospitalized and treated for his injuries.
