Employee Loses Finger When Press Brake Pinches His Hand

At approximately 2:00 p.m. on February 13, 2001, Employee #1 lost his left index finger, lacerated his left middle finger, and lacerated his right index finger when the press brake pinched his hands. He was operating a press brake to form a steel plate. The plate was not centered. Employee #1 flipped the plate over in an attempt to correct the angle of the bend in the steel plate. When the die of the press brake contacted the steel plate, Employee #1 was holding the plate with his hands. The press brake then pinched Employee #1's hands. As a result, Employee #1's left index finger was amputated, his left middle finger was lacerated, and his right index finger was lacerated. He was transported to Kaiser Hospital in Vallejo, California for treatment.
