Investigation Summary
On April 29, 2002, an employee driving a Nissan sit-down forklift ran into a 4-foot-tall yellow metal post. As the forklift ran into the post, the weight of the forklift caused the post to bend back forcing the asphalt/concrete post to break loose. As the asphalt/concrete broke loose, the post bent back acting as a ramp for the forklift's tire. The right front tire drove up the post. This caused the forklift to tip over. The employee attempted to jump clear of the forklift but ended up getting trapped under the roll cage. The employee sustained injuries to his shoulder and chest area and was hospitalized.
