Investigation Summary

Investigation Nr: 201102035
Event: 04/30/2001
Employee falls from an elevation and fractures his leg

On April 30, 2001, Employee #1 was instructed by his supervisor to finish attaching and tightening pipe fittings in the area they had been working together in. In order to do some of the attaching and tightening, he scooted backwards on his stomach so his body was bent at the waist, with his upper torso resting on the roof and his legs hanging down along the wall. He would then reach back with his foot to find the Z-Bar beam, which was 50 inches down and 21.25 inches behind. While he was attempting to move over the edge and reach back with his foot to find the beam, he slipped and fell between 12.5 to 16.66 ft. He was hospitalized with a broken leg.
