Investigation Summary
On July 28, 2004, Employee #1, a tree trimming foreman, and a tree trimmer/cutter, were removing a dead tree that was approximately 30-in. diameter at its base and 65-ft high. Employee #1 had positioned himself in what he thought was a safe zone. When the cutter felled the tree, it struck some smaller trees. A piece approximately 11-ft long snapped off the top of the felled tree, striking Employee #1 in the head, and killing him.
