Employee falls 6 ft from ladder

On May 31, 2002, Employee #1 was standing on top of a 6 ft ladder trying to step onto a ledge. Employee #1 fell 6 ft to the ground sustaining head trauma, a bruised right shoulder, and a bruised right knee. Employee #1 was transported to the hospital and remained there for 4 days.
