Investigation Summary

Investigation Nr: 201083250
Event: 10/05/2001
Employee Injured When Arm Caught In Conveyor Belt

Employee #1 was cleaning conveyor belts on the second-level with a wet rag. Employee #1 observed that the centerline of a belt was dirty and could not be cleaned from the second-level floor where he was working. A metal divider over the centerline of the belt made it impossible to clean. The only area of the belt where the centerline was exposed and could be cleaned was at the very end of the belt that extended outwards from the second level. An unguarded tension roller was located at the end of the belt where Employee #1 began to clean. The rag that Employee #1 was holding became caught in the nip point of the roller and conveyor belt and was pulled in. Employee #1's left hand and arm was subsequently pulled into the nip point. The conveyor motor stopped automatically when the Employee #1's arm became jammed in the roller. Employee #1 suffered fractures to both bones of the forearm, abrasions, and skin damage. Employee #1 was hospitalized.
