Investigation Summary
A concrete form was being moved using a Skytrack with a jib attachment. As the Skytrack was moving, it bounced against the ramp causing the jib attachment to come off and sending the concrete form forward striking Employee #1 on his chest and hip area. Employee #1 suffered fractures and was hospitalized.
