Investigation Summary
Employee #1 broke his ribs when he slipped and fell into a scissors lift. His employer is a mill facility. Employee #1 operates a moulder. He was pulling on a piece of wood, as the pressure bar held the wood down, to get it close enough to the electric eye to make an adjustment. As he pulled the wood, he slipped and fell into the scissors lift roll case. The scissors lift lowered the unit of wood as it was stacking. The electric eyes and limit switches are designed to cause it to move at the correct times. When Employee #1 fell into the scissors lift, it activated the electric eye. The lift then began to rise. Employee #1 became caught between the scissors lift and a metal beam at the side of the scissors lift. The lift lowered when the electric eye was knocked out of adjustment. Employee #1's ribs were broken when the scissors lift began to rise. He went to a local hospital for treatment. .
