Investigation Summary
At approximately 12:30 a.m. on January 31, 2001, Employee #1 was doing general clean up under the conveyor belt line. The belt support roller is 7 ft above the concrete floor and approximately 9 ft long and 3.5 in. wide. The roller is metal and the belt is plasticize canvass. Employee #1 observed a strip of duct tape that had been on the belt had stuck to the roller and the loose end was flapping. Employee #1 showed the piece of flapping tape to his supervisor and tried to grab the end of the tape but missed. The supervisor told Employee #1 to leave the tape alone. The supervisor turned to leave the area and heard Employee #1 grunt. The supervisor turned back and saw Employee #1's arm between the belt and roller. Employee #1 suffered a broken right wrist and arm and was hospitalized.
