Employee Is Burned in Arc Flash

On September 8, 2002, Employee #1 was assigned to assist the head maintenance electrician to pull some cable for a new electrical control box off of the main panel. The main panel for the plant was locked out. There were four cables to be pulled and three had already been pulled. Employee #1 was told to wait until the head electrician was ready to receive the last cable at the bottom of the cabinet and turned around momentarily to pick something up. Employee #1 started to feed the last cable and hit the live side of the buss, 480 volts, and was thrown off the ladder. He received burns to his face, arms, abdomen and fingers. Employee #1 was hospitalized.
