Investigation Summary
Employee #1 is an inmate who was building cabinets from wood. Employee #1 was using a dado blade to make a cut on a raised panel cabinet door. The panel came apart as Employee #1 made the cut. Three of Employee #1's fingers were amputated when they contacted the saw blade. The table saw did not have a guard on the saw blade when Employee #1 was making this cut. A dado blade is not used to make the cut Employee #1 attempted to make. Employee #1 should have changed the saw blade to a single saw blade and not made this cut with the dado blade. Employee #1 was aware that he should use another blade. Employee #1 was hospitalized.
