Investigation Summary
On April 6, 2002, Employee #1, a laborer, was cutting 30-ft sections of 5-in. pipe that were being loaded onto a truck by forklift. The forklift operator struck a piece of pipe on the ground, causing it to roll and strike Employee #1. Employee #1 sustained a fractured ankle and was taken to Hanford Community Hospital for emergency treatment. Employee #1 was hospitalized.
