Employee killed in fall from roof

At approximately 2:30 p.m. on October 9, 2000, Employee #1 and a coworker were roofing a two-story house in Sacramento, CA. It had just begun to rain, and they were spreading a plastic cover over the partially completed wooden shingle roof. Employee #1 was on the east side of the roof and his coworker was on the west side. The coworker had completed his side and had gotten down from the roof when he heard a commotion from the back of the house. When he went around, he found Employee #1 on the ground, bleeding from his head. He had fallen 15 ft 6 in. from the 10-in-12 pitched roof. There were roof jacks and toe boards on much of the roof, but he had slid down an approximately 3 ft gap between two protected areas. Employee #1 sustained severe head injuries and died a short time later.
