Investigation Summary
On March 20, 2002, a plant mechanic with San Rafael Rock Quarry was operating an aerial lift while traveling down a grade steeper than that recommended by the equipment manufacturer. The aerial lift apparently went into overspeed, and the employee lost control, turning sharply and bouncing over a 5-foot embankment. The employee was thrown into the air, and, restrained by a harness and lanyard, was slammed back into the man basket. The employee suffered lacerations, bruises and closed skull trauma. The employee was transported by ambulance to John Muir Hospital in Walnut Creek, California and was hospitalized for concussion.
