Employee loses balance and falls from ladder

At approximately 1:00 p.m. on July 23, 2004, a carpenter was removing an extension ladder from a scaffold. He removed the wiring that had secured the ladder. The employee then descended the ladder. He attempted to lower the ladder, but the ladder was still partially attached to the scaffolding. The employee climbed the ladder and clipped the remaining wires. The ladder began to slide because the securing arms on the ladder had become disengaged from the locked position. As a result, the employee lost his balance and fell approximately 15 feet to the concrete floor below. The employee sustained serious injuries, and he was hospitalized.
