Employee severs tip of finger

On January 29, 2004, Employee #1, an equipment mechanic at a corporate yard, was working on a tractor mount blower to repair and prepare it for use. The blower was not energized and Employee #1 had placed the blower hood on the equipment. Next he rotated the blade to check for clearance, making sure that the blade did not hit the cover during movement. Then he placed an 8-inch metal punch in place of the PTO shaft in order to move the blade to check for clearance. As he rotated the shaft in a clockwise direction, the 8-inch punch slipped out of the hole in the shaft and crushed his left ring finger between the punch and a support beam of the blower. Employee #1 lost 0.5 inches off the top of his left ring finger.
