Employee's hand amputated when caught in a machine

On November 26, 2003, Employee #1 working for an employment agency, was working at a newspaper printing facility stacking bundles of papers as they came out of a banding machine. He reached to pick up a bundle of papers and inadvertently placed his fingers into an opening by the tail drum of the belt conveyor. The opening was approximately four inches wide and his fingers were caught between the belt and the drum. His hand was amputated up to his wrist and he was hospitalized.
