Employee injured while operating conveyor belt system

On April 1, 2004, Employee #1 was operating a conveyor belt system. For some reason, a plastic bag became stuck in the conveyor belt. As employee #1 reached under conveyor belt to retrieve the plastic bag, her hand became caught between the belts. Employee #1 suffered a contusion on her left hand.
