Employee Is Injured When Pinned by Machine

At approximately 9:30 a.m. on March 20, 2004, Employee #1, a technician, was instructed by her supervisor to perform a quality check on the glass being stacked by an edge-delete stacker. She placed the stacker in "HOLD" mode and then walked to the access door, but it was already open. After walking through the gate, Employee #1 observed the stacker had placed one more piece of glass. She crouched down on the glass pallet to look for cracks, when the machine suddenly restarted its stacking cycle. The machine attempted to stack a piece of glass on top of her and as a result, she was pinned between the stacked glass and the piece of glass being stacked. The stacker automatically stopped and became jammed in place. Employee #1 was pinned for over an hour. She was hospitalized for 10 days due to bruising and internal injuries. A coworker, the stacker operator, stated that on previous occasions placing the machine in "HOLD" mode cycled out the stacker and unlocked the gate rather than opening it.
