Employee Breaks Arm Reaching Through Mast of a Forklift

On February 27, 2004, Employee #1 of Blt Ent. Dba Sacramento Recycling & Transfer was reaching over controls and through the mast on a forklift, attempting to remove metal chain that was wrapped around the mast/forks of the forklift. Employee #1 accidently activated the controls and his sleeve on right arm got caught as the mast/forks were descending. Employee #1 sustained broken right arm in the event.
