Investigation Summary
On August 25, 2003, a truck driver was working for a logging company and was operating a water truck. The truck placed water on the logging roads to control dust. The employee was driving down a mountain road. He discovered he was unable to slow the truck or stop it. The employee steered the truck into the bank of the road to stop the truck. The truck tipped onto its side and stopped after sliding on the roadway. An investigation of the accident revealed that the driver did not keep his brakes properly adjusted. The brakes were too loose and unable to stop the truck. The employee was hospitalized with a fractured left arm and an injured lung.%09
