Employee's Leg Crushed by Banding Machine

Employee #1, working for Sierra Pacific Industries in Arcata, CA, was operating a banding machine to manually band metal to a unit of lumber for shipping and handling. Employee #1 was kneeling on the unit of lumber with his left leg on the left side of the unit. The banding machine squeeze ram activated unexpectedly and crushed Employee #1's left lower leg. Employee #1 was hospitalized and his left leg was amputated.
