Investigation Summary

Investigation Nr: 170866644
Event: 11/12/1999
Employee's fingertips amputated when caught in rollers

At approximately 8:00 a.m. on November 12, 1999, Employee #1, a printing feeder, was wiping down the blanket cylinder of a Miehle T-3 off-line coater press, a machine that applies a UV coating to printed cartons. He was holding a folded terrycloth rag in the palm of his gloved right hand, wiping the in-running cylinders when his fingers became caught between the two moving rolls. The tips of his ring and little fingers were amputated. He managed to pull out his hand, and he was taken by private car to Mercy Hospital, Sacramento. Employee #1 was later transferred to California Pacific Medical Center in San Francisco, where attempts to reattach his fingertips were unsuccessful.
