Employee's arm fractured between bar and palletizer frame

Employee #1 was reaching into a can line palletizer to remove a stick of wood when the squeeze bar on the left side returned to its nesting position and caught his right forearm against the machine frame, fracturing it. The palletizer was not turned off when he reached inside. Employee #1 admitted that the accident was his fault, as he had been trained in lockout and tagout procedures.
