Investigation Summary
The employer, Allied Technology Group, Inc., was general contractor for the store renovation work in San Ramon. Employee #1 was the site supervisor. Just before the accident, Employee #1 and a carpenter were lifting a 6-inch by 8-inch by 16-foot-long wood beam into place by means of two separate 8 foot stepladders. When Employee #1 reached the second step from the top, he lost his balance and fell onto the concrete floor. Employee #1 sustained multiple skull fractures and expired three days later at John Muir Medical Center in Walnut Creek.
