Employee's thumb struck by lid

On April 8, 2004, Employee #1 was assisting a truck mounted crane to place a 400 pound steel lid onto a cement vault. The vault was in the ground. One of the 2-ton lifting devices slipped off when the lid was being lowered and lacerated the employee's right thumb. Employee #1 was not hospitalized.
